Applications to apply for or renew a Video Gaming Terminal Operator, Establishment, Manufacturer, or Supplier license are provided below.
Please Note: All applications for gaming employee permits associated to the license types listed above must be submitted through the SLOTSlink online electronic application. For access to SLOTSlink, please contact your Human Resources Office.

Video Gaming Terminal (VGT)
- Enterprise Entity Application and Disclosure Information Form
- Entities applying as an Operator, Manufacturer, Manufacturer Designee, Supplier, or an intermediary or holding company of those license types should complete this form.
- Abbreviated Application for Licensed Manufacturers to apply for a Video Gaming Manufacturer License
- Entities currently licensed as a Manufacturer with the Pennsylvania Gaming Control Board may complete this form.
- Abbreviated Application for Licensed Suppliers to apply for a Video Gaming Suppliers License
- Entities currently licensed as a Supplier with the Pennsylvania Gaming Control Board may complete this form.
- Multi-Jurisdictional Personal History Disclosure Form
- Individuals applying as a Principal/Key Employee must complete this form.
- Principal/Key Employee Form-Pennsylvania Supplement to the Multi-Jurisdictional Personal History Disclosure Form
- Individuals applying as a Principal/Key Employee must complete this form.
- Principal Entity Form
- An entity that is otherwise required to be licensed as Principal and is not an intermediary or holding company of an applicant or license must complete this form.
- Abbreviated VGT License RENEWAL Application
- This abbreviated VGT license renewal application may be used by an entity or individual currently licensed with the Board as an Affiliate, Principal Entity, Principal, or Key Employee and they hold other non-VGT licenses that are not required to renew at this time.

- Licensee Notification of Proposed Transfer of Interest Form
