All applications, submissions, and filings made by, on behalf of, or concerning Hacksaw Gaming US LLC and Hacksaw Gaming PA (and any affiliated or related “Hacksaw” entity that filed under a variant name, including “Hacksaw Gaming,” “Hacksaw Studios,” or any successor or parent entity), in connection with licensure, certification, registration, permitting, or approval by the Board, including but not limited to interactive gaming manufacturer, interactive gaming supplier, manufacturer, supplier, and gaming service provider (certified or registered) categories:
1. All license, certification, and registration applications and application forms submitted by or on behalf of the above entities, including the completed application packets and any amendments, supplements, or renewals thereto.
2. All financial statements submitted as part of, or in support of, those applications — including audited and unaudited financial statements, balance sheets, income (profit and loss) statements, statements of cash flows, statements of changes in equity, annual reports, and any financial statements of parent, holding, or affiliated companies filed in connection with the applications.
3. All exhibits, schedules, attachments, disclosure forms, and supporting documentation filed with or referenced by the applications (for example, organizational charts, ownership and control disclosures, and good-character/suitability submissions), excluding any personal identifying information that the Board is required to withhold.
4. All correspondence, emails, and written communications between the Board (including the Bureau of Investigations and Enforcement and the Office of Enforcement Counsel) and the above entities or their licensed entity representatives concerning the applications.
5. All Board orders, resolutions, notices, staff reports, recommendations, and public meeting or agenda materials approving, conditioning, denying, or otherwise disposing of the applications.
6. Any application status, disposition, or determination record reflecting the current standing of each entity’s application(s) with the Board.
