Machine Data: All data recorded by Slot Machine N2204 / Asset #1 no on August 2nd, 2024, particularly between 9:00 PM and 11 :00 PM when a malfunction occurred. 

The Enterprise Entity Application and Disclosure Information Form submitted by H. Betti Industries, Inc. in conjunction with its application for a video gaming terminal supplier license (GID#2171-2).

The following pertaining to Jeff Yass and/or Michael Gary Rubin in association with their license as principal for PointsBet:

1) The non-confidential portions of the personal history disclosure (PHD)

2) A redacted and/or only initial cover page for the submission

3) A simple record/document stating the PHD submission was submitted/received for the individuals as this is non-confidential and legally required for principals

Provide the start and end dates for the use of each alias by the employees listed in the previous request, including both alias names and real names.
